*NOTICE: Once a patient is accepted by the providers at Mindful Matters, the new patient packet MUST be submitted at least two business days prior to the initial evaluation appointment.

Forms may be faxed, physically dropped off at our office location, and/or sent via mail. However, forms MAY NOT be completed and returned via e-mail due to protection of Personal Health Information (PHI).
